I understand that providing incomplete, false, or misleading information may disqualify me from employment consideration or result in termination if discovered after hire. I authorize Mesa Verde Enterprises, Inc. and/or Aggregate Technologies, LLC (MVE/AGGTEC) to investigate all statements made in this application as necessary to make an employment decision. I understand that MVE/AGGTEC conducts post-offer background checks relevant to job duties. As part of the background screening process, the Company may conduct verification of prior employment; review publicly available New Mexico court records; search the National Sex Offender Registry; perform motor vehicle record (MVR) checks; and verify internal rehire eligibility. I also understand that applicants may be required to complete pre-employment drug and alcohol screening and a physical examination as conditions of employment. These evaluations will be scheduled and paid for by MVE/AGGTEC, and the results will be the property of MVE/AGGTEC. I acknowledge that acceptance of a conditional offer of employment is contingent upon the successful completion of all required screenings. Failure to successfully complete the required screening and/or physical examination will render me ineligible for employment.
